Job summary

PATH (People Assisting The Homeless) is seeking a Case Manager for the Noho Senior Villas Office in Los Angeles. This role supports clients through homelessness housing stages with a client-centered, flexible approach.

You will conduct intake, develop ISP plans, coordinate housing resources, and maintain documentation. The position is full-time, Monday–Friday, with growth opportunities within PATH.

Responsibilities

• Conduct intake and assess participants to develop an ISP addressing housing goals and supports.
• Provide referrals and connections to resources to help achieve ISP goals.
• Complete program-specific assessments (e.g., VI-SPDAT).
• Evaluate strengths and challenges; conduct 90-day reviews with participants.
• Maintain documentation and ensure complete, accurate participant files.
• Maintain confidentiality per HIPAA and guidelines; build relationships with clients and partners.
