Job summary

PATH (People Assisting The Homeless) is seeking a Case Manager for the Site Base Housing team in Los Angeles, CA. You will conduct intake, develop individualized service plans, coordinate housing and health services, and provide ongoing support to help clients achieve housing stability.

This role emphasizes trauma-informed care, crisis intervention, and collaboration with community partners to address barriers and improve outcomes for clients experiencing homelessness.

Qualifications

• All levels of education and experience are welcome.
• Two years of experience in healthcare, case management, homeless services or related field preferred.
• Mandatory employment eligibility; CPR/First Aid training may be required.

Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.
• Provide referrals and linkages to services to meet ISP goals.
• Complete program assessments (e.g. VI-SPDAT).
• Maintain confidential records and documentation per HIPAA and PATH policies.
• Develop therapeutic relationships to support housing goals and safety.
