Job summary

PATH (People Assisting The Homeless) is hiring a CalAIM Community Support Case Manager - Housing Navigation for our San Jose, CA office. This role focuses on helping clients break the cycle of homelessness through a flexible, client-centered approach that supports housing goals from the street to permanent housing.

You will conduct intake, develop individualized service plans, connect clients to resources, complete program assessments, and maintain thorough case files, all while upholding PATH’s

Qualifications

• Minimum two years of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field.

Responsibilities

• Conducts intake and individualized needs assessment for participants and develops ISP addressing housing goals and income.
• Provides referrals and connections to resources to meet ISP goals.
• Completes program assessments (e.g., VI-SPDAT) and maintains documentation per PATH policies.
• Evaluates strengths and challenges to address short- and long-term housing goals and conducts 90-day reviews.
