Job summary

National Health Foundation seeks a Housing Navigator in Los Angeles to assist participants facing housing crises through intensive case management and housing stability planning. The role collaborates with housing authorities, DMH, and landlords to secure permanent housing and subsidies.

A BA or AA in related field is preferred, with required CPR/First Aid and a valid driver’s license. Background checks and TB tests are required.

Qualifications

• BA degree preferred.
• Associate degree preferred with minimum three years in social services.
• CPR/First Aid Certification required.
• Reliable transportation and valid driver’s license with insurance.
• Experience with affordable housing programs (Section 8, VASH etc.).
• Ability to work autonomously and in multidisciplinary teams.
• Comfort providing field-based services at motels/interim housing locations.

Responsibilities

• Provide direct housing crisis assistance and intensive case management.
• Develop and implement housing plans and participant budgets.
• Coordinate with Housing Authority, DMH, and landlords to secure housing.
• Enter data, track services, and maintain guest eligibility documentation.
• Conduct initial assessments and follow-up visits to move participants toward permanent housing.
• Follow NHF policies and procedures.
