PATH is seeking a Case Manager for our North Long Beach office to support clients experiencing homelessness. You will conduct intake, develop ISP plans, provide referrals, complete assessments, and maintain documentation, aiming to help participants secure housing and stabilize their lives.

Qualifikationen

• Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field.
• All levels of education and experience are welcome.

Aufgaben

• Conducts intake and individualized needs assessment for all participants and collaborates to develop ISP addressing housing goals, income and personal goals.
• Provides referrals, linkages, information, and support to resources that help participants achieve ISP goals.
• Completes program specific assessments (e.g., VI-SPDAT).
• Evaluates strengths and challenges to addressing short and long-term goals; conducts a 90 day review with the participant after intake.
• Maintains documentation standards including case file notes; keeps an accurate participant file.
• Maintains confidentiality of participant files per HIPAA and guidelines.
• Develops therapeutic and professional relationships to engage and retain clients in housing goals.
• Provides crisis intervention and risk assessment focused on safety and independence in care.
• Uses motivational interviewing and trauma-informed care in service delivery.
• Maintains knowledge of community resources to provide wrap-around services.
