Job summary

ServiceSource in Clearwater, FL is seeking a dedicated Case Manager to support residents with disabilities and barriers to affordable housing. You will assess needs, develop individualized service plans, and coordinate essential services to promote independent living and successful tenancy.

Knowledge of trauma-informed, person-centered care and strong documentation skills are essential. Collaboration with housing staff and community partners is required to ensure comprehensive support for

Qualifications

• Bachelor’s degree in Social Work, Psychology, Business, or a related field is required.
• Licensed Social Worker (LSW) strongly preferred.
• Minimum of 1 year of case management experience.
• Valid driver’s license with good driving record.

Responsibilities

• Conduct resident intake, assessments, and ongoing needs evaluations.
• Develop individualized service plans with goals, barriers, strengths, referrals, and measurable steps.
• Provide direct case management, advocacy, and supportive counseling within scope of practice.
• Maintain accurate, timely, confidential case notes and program documentation.
• Coordinate referrals to healthcare, housing, benefits, transportation, and other services.
• Collaborate with housing staff and community partners to support resident well-being.
• Prepare reports and updates for management, funders, and regulatory expectations.
