ServiceSource Inc. seeks a dedicated Resident Services Case Manager to support low‑income residents, including those with disabilities, in achieving stable housing and improved life quality.

Primary Duties Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions. Conduct resident intake, assessments, and ongoing needs evaluations for low-income residents, including individuals with disabilities and residents experiencing barriers to housing stability, healthcare access, transportation, employment, benefits, or daily functioning. Develop individualized service plans that identify goals, barriers, strengths, referrals, and measurable steps to support independent living, successful tenancy, and improved quality of life. Provide direct case management, crisis intervention, resident advocacy, and supportive counseling within scope of practice to assist residents with problem-solving, adjustment, and access to needed services and support. Maintain accurate, timely, and confidential case notes, referral records, service plans, outcomes tracking, and program documentation in required systems. Coordinate referrals to community-based resources including healthcare, behavioral health, disability services, benefits counseling, transportation, legal aid, food access, vocational support, and other supportive services.
Additional Responsibilities Provide comprehensive case management services, maintain and update resource directories, educate residents on available services and tenant responsibilities, coordinate with community partners and housing staff, support resident engagement activities, and perform other responsibilities as assigned. Collaborate with affordable housing staff, property management, and community partners to support resident well-being while maintaining appropriate professional boundaries and confidentiality. Prepare reports, case summaries, resident service updates, and program documentation as required by management, funders, regulatory expectations, and organizational policy. Qualifications: Education, Experience, and Certification(s) Bachelor’s Degree (BA or BS) in Social Work, Psychology, Business, or a related field required. Licensed Social Worker (LSW) strongly preferred. At least one (1) year of case management experience required. Valid driver’s license and good motor vehicle record required.
Knowledge, Skills, and Abilities Knowledge of case management principles, person-centered practices, trauma-informed approaches, supportive service delivery, affordable housing programs, community resources, disability-related services, entitlement and public benefit systems, reasonable accommodations, confidentiality requirements, professional boundaries, crisis response, and ethical responsibilities associated with case management practice. Strong organizational, documentation, computer, verbal, and written communication skills, including proficiency in Microsoft Office and the ability to learn property management, case record, and other organization-specific software.

While performing the duties of this job, the employee is regularly required to communicate, use standard office equipment, and move about in an office environment. The employee may occasionally stand, walk, reach, bend, or lift materials weighing up to 20 pounds.
