Job summary

Catholic Housing Services in Seattle, WA seeks a full-time Case Manager to support residents through intake, planning, and ongoing housing services. You will coordinate access to community resources, advocate for residents, and document services using HMIS.

Responsibilities include developing individualized housing plans, engaging residents, and ensuring compliance with housing quality standards. Strong communication and crisis intervention skills are essential.

Qualifications

• Demonstrated skills in problem solving, issue assessment and crisis intervention.
• Ability to communicate effectively, verbally, and in writing while maintaining a professional demeanor.
• Experience working in residential facilities or understanding of residential settings.
• Basic computer skills to maintain client records and create informational materials.
• Ability to work independently as well as in a team environment.

Responsibilities

• Complete new resident intake and case management plans, assess needs and link to resources.
• Develop a Housing Service Plan addressing residents' identified needs.
• Engage residents through outreach and group activities to promote a healthy community.
• Provide case management, access benefits, and facilitate service access for residents.
• Maintain documentation in resident files and HMIS reporting, including contracts and referrals.
• Assist with appointments, transportation, and accompany residents when needed.
• Conduct home visits to verify housing conditions and safety compliance.
• Complete monthly/quarterly reporting and maintain client files per funder requirements.
