Qualifications

• Bachelor’s degree in social work, psychology or related field.
• Or high school diploma with 12 college credits in social sciences and 2 years mental health direct care.
• Ability to communicate verbally and in writing with all levels of the behavioral health system.
• Experience with crisis intervention and respect for participant rights.
• Proficient with Outlook, Word and Excel; able to maintain daily productivity.

Responsibilities

• Provide engagement and case management support, including home-based visits in the Philadelphia community.
• Interact with a multidisciplinary team to ensure individualized services for participants.
• Assist participants toward long-term housing while complying with agency regulations and policies.
