Job summary

Eden Housing seeks a Case Manager to oversee resident services programming across properties serving high-need populations, including seniors and special-needs individuals. You will coordinate services with county providers, build community partnerships, and ensure compliance with HIPAA policies while promoting independent living and resident well-being.

Under supervision, you will deliver programs, conduct outreach, and support residents in achieving personal and functional goals.

Qualifications

• Excellent reading, writing and verbal communications
• Ability to navigate language barriers using translation tools
• Experience serving high-need and/or homeless populations
• Experience with data entry and report generation
• Knowledge of HIPAA policies and procedures

Responsibilities

• Deliver onsite programs and ensure they meet standards and regulatory expectations
• Conduct outreach to residents via door-to-door, phone, and email
• Provide crisis intervention and follow mandated reporting policies
• Support case management for residents with functional impairments
• Coordinate with community partners and service providers to deliver programming
