Job summary

Wyandot Behavioral Health Network seeks a dedicated Case Manager to join our community-based team serving adults with severe mental illness. You will build relationships, coordinate services across health providers, connect clients to housing, healthcare, and community resources, and advocate for their wellbeing.

Responsibilities include assessments, treatment planning, crisis support, and ensuring high-quality care with compliant documentation and teamwork. Travel in the community is required.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Rehabilitation, Human Services, or related field.
• Valid driver’s license, reliable transportation, and current automobile insurance.
• Strong interpersonal, communication, organizational, and time management skills.
• Ability to work independently while collaborating within a multidisciplinary team.
• Basic computer proficiency and the ability to use electronic health records.

Responsibilities

• Develop trusting, recovery-focused relationships with individuals and families.
• Conduct strengths-based assessments and collaborate on person-centered treatment plans.
• Coordinate care with behavioral health providers, primary care, hospitals, and community organizations.
• Connect clients to housing, healthcare, financial assistance, employment, transportation, and resources.
• Advocate for clients with healthcare providers, courts, schools, government agencies, and social services.
• Support individuals during crises by coordinating interventions and services.
• Monitor progress and adjust service plans as needed.
• Complete timely, accurate documentation meeting Medicaid, agency, and regulatory standards.
• Participate in team meetings, clinical supervision, ongoing training, and professional development.
• Travel throughout the community to meet clients and transport them to appointments.
