Job summary

Talin Home Healthcare LLC in Sterling, VA is seeking a motivated and detail-oriented Home Health Intake Coordinator to join our growing team. You will be the first point of contact for patients and families, helping to connect them with compassionate care from day one in a fast-paced healthcare environment.

Responsibilities include managing end-to-end intake, verifying insurance, obtaining authorizations, coordinating with clinical staff, and maintaining EMR records.

Qualifications

• High school diploma or equivalent required; healthcare-related associate's degree preferred.
• Strong organizational and multitasking abilities to manage multiple referrals.
• Excellent verbal and written communication and compassionate patient care approach.
• Experience with EMR/EHR systems and insurance authorizations is preferred.

Responsibilities

• Manage the end-to-end patient intake process, including referrals from hospitals, physicians, and discharge planners.
• Verify patient insurance eligibility and obtain authorizations for home health services.
• Coordinate with clinical staff to ensure timely start-of-care for new patients.
• Maintain accurate and up-to-date patient records in the EMR/EHR system.
• Communicate effectively with patients, families, referral sources, and internal care teams.
• Ensure compliance with Medicare, Medicaid, and private insurance guidelines for home health admissions.
• Track and follow up on pending referrals and authorizations to minimize delays in care.
