Job summary

Castor Home Health is seeking a full-time LPN Intake Coordinator in Sterling, IL to manage referrals from receipt through admission. This role works closely with patients, families, referral sources, clinical leadership, and scheduling to ensure timely and smooth starts of care.

You will review referrals, verify information, coordinate insurance requirements, and support admissions while delivering a positive patient experience.

Qualifications

• Current, unrestricted Illinois LPN license.
• Healthcare intake, home health, care coordination, or related experience preferred.
• Knowledge of home health referral and admission processes is a plus.
• Strong communication, organization, and follow-through.
• Ability to manage multiple priorities in a fast-paced environment.
• Experience with an electronic medical record system is preferred.

Responsibilities

• Receive and review new home health referrals.
• Contact patients and families promptly to confirm interest and readiness for services.
• Review referral documents and obtain missing orders or clinical records.
• Coordinate insurance verification and authorization requirements.
• Communicate with physicians, hospitals, facilities, and other referral sources.
• Work with clinical leadership to address eligibility or clinical questions.
• Coordinate accepted referrals with the scheduling and field teams.
• Track pending referrals, follow up on delays, and elevate barriers.
• Maintain accurate referral statuses and documentation.
• Support timely admissions and a positive patient experience.
