Job summary

One Health Home Health is currently seeking an experienced Intake Coordinator to join our team delivering high-quality home-based patient care. You will evaluate referrals, confirm insurance coverage, and coordinate services with clinicians to ensure a seamless continuum of care.

The role requires communicating with physicians, nurses, social workers, and discharge planners, obtaining prior authorizations, and handling fast-paced situations with compassion.

Qualifications

• Minimum of one year of experience in intake coordinating.
• Ability to multi-task in a fast-paced environment.
• Excellent written and verbal communication skills.
• Knowledge of home health management and insurance limitations.
• Fluent English, both verbal and written.

Responsibilities

• Evaluates patients referred, identifies insurance coverage, and coordinates referrals for services.
• Works with physicians, nurses, social workers and discharge planners to establish a coordinated plan of care.
• Obtains and documents prior authorization for services from insurance providers.
• Demonstrates compassion and courtesy in difficult situations.
• Able to work under pressure including overtime to complete daily assessments.
• Uses sound judgment and a tactful approach with coworkers, staff and the public.
• Applies clinical knowledge to the referral process.
