Job summary

Talin Home Healthcare LLC in Sterling, VA is seeking a motivated Home Health Intake Coordinator to be the first point of contact for patients and families, helping connect them with compassionate care. You will manage referrals from hospitals and physicians and coordinate with clinical staff to start care promptly.

Onsite role requiring 30–40 hours per week, with emphasis on accurate EMR/EHR documentation, regulatory compliance, and excellent communication with patients and teams to ensure

Qualifications

• Strong verbal and written communication skills.
• Excellent organizational abilities with multi-referral management.
• Compassionate, patient-focused demeanor and commitment to quality care.
• Knowledge of Medicare/Medicaid home health regulations and insurance authorizations.

Responsibilities

• Manage end-to-end patient intake process from referrals.
• Verify insurance eligibility and obtain authorizations.
• Coordinate with clinical staff to ensure timely start-of-care.
• Maintain accurate patient records in EMR/EHR system.
• Communicate with patients, families, sources and internal teams.
• Ensure compliance with guidelines for home health admissions.
• Track referrals and authorizations to minimize delays in care.
