Job summary

Center of Illinois Home Health Care, Inc. in Arlington Heights, IL is seeking a Home Health Intake Coordinator to manage referrals, verify insurance benefits, obtain authorizations, and coordinate admissions.

This role acts as the liaison between referral sources, patients, clinicians, and agency leadership. The successful candidate will handle scheduling coordination, maintain accurate intake records, and ensure HIPAA compliance while providing exceptional customer service in a full-time,

Qualifications

• High school diploma or GED required.
• Minimum of 2 years of home health experience preferred.
• Experience with home health intake, referrals, insurance verification, or healthcare administration preferred.

Responsibilities

• Manage daily patient referral and intake operations.
• Verify insurance eligibility, benefits, and authorizations.
• Coordinate admissions and scheduling of clinical staff.
• Maintain accurate intake records and documentation.
• Build and maintain positive relationships with referral sources.
• Ensure compliance with agency policies and regulatory requirements.
• Assist referral sources with information regarding available services and community resources.
• Track physician orders and obtain signed documentation as required.
• Communicate professionally with patients, families, physicians, referral partners, and team members.
• Answer incoming phone calls and direct inquiries appropriately.
• Maintain patient confidentiality and HIPAA compliance.
• Participate in quality improvement initiatives and agency training programs.
• Provide administrative support to agency leadership as needed.
