Responsibilities

• Gathers data for all relevant assistance programs face-to-face or via phone with clients.
• Verify information meets state policies; obtain releases and medical records as needed.
• Verify income, assets, and other financial documents; data entry into state database.
• Respond to inquiries by phone or email; explain program requirements.
• Utilize automated program to determine eligibility for state assistance programs.
