Qualifications

• Proficient in Microsoft Office; reliable self-starter.

Responsibilities

• Gathers data for all relevant assistance programs face-to-face or via phone with clients
• Verifies information is gathered accurately and meets state policies
• Obtains release forms and medical records as needed
• Verifies income, assets, and other financial / legal documents
• Reviews regulations and program requirements with clients
• Utilizes automated program to determine eligibility of state assistance programs; data entry into state database
• Responds to all inquiries via phone or email
