Job summary

PATH is seeking a Case Manager for its Villas South Park office in Los Angeles, CA. The role supports individuals experiencing homelessness through intensive case management, ISP development, and housing placement efforts.

You will conduct intake, assess needs, coordinate referrals, and monitor progress across housing stability, income goals, and health services, following PATH policies and maintaining documentation.

Responsibilities

• Conduct intake and individualized needs assessment for all participants.
• Develop and monitor ISP addressing housing goals and income goals.
• Provide referrals and linkages to resources supporting ISP goals.
• Complete program-specific assessments (e.g., VI-SPDAT).
• Maintain documentation per PATH policies and case notes.
