Job summary

PATH (People Assisting The Homeless) is hiring a Case Manager/Housing Navigator in Los Angeles, CA. The role supports a Housing Navigation Team through an evidence-based, whatever-it-takes approach to break the cycle of homelessness.

Responsibilities include intake, ISP development, referrals, VI-SPDAT, and ongoing client file maintenance with HIPAA compliance, while contributing to PATH's mission and racial and social justice goals.

Responsibilities

• Conduct intake and needs assessments to develop ISP addressing housing goals and income.
• Provide referrals and support to help clients meet ISP goals.
• Complete program assessments (e.g. VI-SPDAT) and track progress.
• Review progress with clients and adjust goals as needed.
• Maintain complete, accurate, and confidential client files.
• Ensure HIPAA compliance and PATH policy adherence.
