Job summary

The Salvation Army is seeking a part-time, non-exempt Case Manager at its 360 Life Center in Illinois. You will perform outreach, intake, needs assessment, direct assistance, and discharge planning, developing case plans and following up with clients to ensure ongoing support.

You will coordinate with funding partners, maintain HMIS/SIMS data, and refer clients to community resources while upholding privacy and program standards.

Qualifications

• Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience.
• 3–5 years knowledge and work experience related to Social Services and Case Management preferred.
• Familiarity with Continuum of Care and homeless service providers.

Responsibilities

• Conduct client intake interviews to determine eligibility for programs or agency referrals.
• Provide case management services including needs assessment, direct assistance, and case plan development.
• Act as liaison to community resources and arrange client transportation.
• Ensure access to a case manager through appointments and walk-ins; meet with clients regularly.
• Document client information and progress in HMIS/SIMS or other required databases.
• Record, track and submit statistical information to comply with funders and DHS/HUD.
• Attend monthly Continuum of Care (COC) meetings and advocate for clients.
• Maintain client records with confidentiality and uphold program policies.
