Qualifications

• Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred.
• Three to five years knowledge and work experience in social services and case management preferred.
• Strong written and verbal communication skills; ability to work with diverse populations.

Responsibilities

• Conduct client intake to determine eligibility for programs and referrals.
• Provide case management including needs assessment, case plan development and follow-up.
• Maintain client records ethically and confidentially and document progress in HMIS or similar systems.
• Coordinate referrals to community resources and assist with transportation as needed.
• Attend meetings and ensure compliance with funding requirements (DHS, HUD, United Way, FEMA).
