Job summary

The Salvation Army North and Central Illinois Division is seeking a Part-Time Case Manager for the Central Illinois Area Command, Canton 360 Life Center. You will conduct client intake, assess needs, develop case plans, provide direct assistance, and coordinate referrals with funding partners.

Qualifications

• Bachelor’s degree or significant related experience in social services preferred.
• Experience in case management and working with homeless service providers preferred.
• Ability to work with diverse populations and maintain confidentiality.

Responsibilities

• Conduct client intake interviews to determine eligibility for programs or referrals.
• Provide case management services including assessment, plan development, and follow-up.
• Coordinate referrals and transportation to ensure access to services.
• Maintain client records and progress in HMIS/SIMS or equivalent systems.
• Attend meetings and collaborate with community partners.

Education

Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience may be considered in lieu of a degree
