Job summary

The Salvation Army North and Central Illinois Division is seeking a Part-Time Case Manager to provide outreach, intake, and assessment, develop client case plans, and coordinate direct assistance with community resources in Central Illinois (Kankakee 360 Life Center).

The successful candidate will manage client eligibility, discharge planning, and follow-up, ensuring compliance with funding partners and agency standards while maintaining ethical, confidential records.

Qualifications

• Bachelor's degree in social services or a related field preferred
• 3–5 years knowledge and work experience in social services and case management preferred
• Familiarity with local resources and homeless services providers

Responsibilities

• Conduct client intake interviews to determine eligibility for programs or referrals
• Provide case management services including assessment of client needs, provision of direct assistance, and case plan development
• Coordinate liaison to community resources and referrals to appropriate agencies
• Arrange transportation for clients and ensure access to a case manager through appointments or walk-ins
• Document client progress in HMIS/SIMS or other required databases
• Record and submit statistical information to maintain funding compliance
• Attend monthly Continuum of Care meetings and participate in program outcomes
• Maintain confidential client records and uphold program policies and mission
