Job summary

The Salvation Army North & Central Illinois Division in Central Illinois Area Command, Galesburg 360 Life Center is seeking a part-time Case Manager to provide outreach, intake, needs assessment, direct assistance, and discharge planning. You will develop case plans and coordinate referrals to community resources.

The role requires 3–5 years of social services experience, a bachelor’s degree in a related field, and proficiency with HMIS or WellSky SIMS.

Qualifications

• Three to five years knowledge and work experience in social services and case management preferred.
• Familiarity with local resources, Continuum of Care and homeless service providers.
• Proficiency with case management databases and MS Office applications.

Responsibilities

• Conduct client intake interviews to determine program eligibility.
• Provide case management including needs assessment, direct assistance, and case plan development.
• Coordinate referrals to community resources and track progress in HMIS/WellSky.
• Ensure clients have access to case management through appointments or walk-ins.
