Job summary

The Salvation Army North and Central Illinois Division is seeking a part-time Case Manager in Canton, IL 360 Life Center. You will perform outreach, intake, assessment, direct assistance, and discharge planning while coordinating with funding partners to meet program requirements.

Responsibilities include maintaining client records, updating databases, and providing referrals and transportation arrangements as needed, with a focus on dignity and confidentiality.

Qualifications

• Bachelor's degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience in lieu of a degree.
• Three to five years of knowledge and work experience related to social services and case management is preferred.
• Familiarity with local resources, Continuum of Care and homeless service providers.

Responsibilities

• Conduct client intake interviews to determine eligibility for programs or agency referrals.
• Provide case management services including needs assessment, direct assistance, case plan development and compliance.
• Discharge planning and follow-up care with resource referrals and eligibility screening.
• Maintain HMIS/SIMS data entry and ensure compliance with funders.
