Humana is seeking a Case Manager to serve members in Wisconsin Family Care, covering Rock and/or Jefferson counties. You will conduct assessments, develop MCPs, and coordinate with a Field Care Nurse.

This role requires a Bachelor's degree in human services or related field with 1 year of relevant experience, or a different degree with 3 years of relevant experience, and travel up to 40% with a valid driver’s license. Typical hours are Monday–Friday with travel reimbursements.

Qualifications

• Bachelor's degree in human services or related field with 1+ year of relevant experience.
• Or a Bachelor's in another field with 3+ years of relevant experience.
• Ability to travel up to 40% with valid driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsabilités

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with Field Care Nurse to support MCP in the least restrictive environment.
• Travel to members' residences for assessments and quarterly visits.
• Arrange non-medical services such as housing, employment, and social integration.
• Maintain accurate documentation including MCP updates.
