Job summary

Care Lync is seeking a full-time Case Manager to provide the activities necessary to meet the needs of the clients assigned to the consolidated care team. If you are highly organized and detail-oriented, you can make a difference working for a social services company that truly cares about those it serves.

In this role you will coordinate care across physical, behavioral, and support services, develop annual plans, and build strong relationships with participants and their families.

Qualifications

• BS/BA degree in related course work.
• No prior experience required.
• Experience with Microsoft Office, state-maintained databases.
• Must maintain proficiency in company sponsored training and certifications.
• Maintain updated state related Clearances
• Successfully complete and maintain training courses as required or amended by program regulations.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration, copies must be maintained on file with Care Lync.

Responsibilities

• Discerning the disabilities and accompanied needs of participants and helping them achieve their goals.
• Develop and modify care plans annually or with significant changes.
• Coordinate with physical, behavioral, and support services to ensure seamless delivery.
• Develop and maintain professional relationships with participants and families.
