Job summary

Care Lync is seeking a dedicated Support Coordinator / Service Coordinator to develop individualized care plans and coordinate services for adults with Intellectual, Developmental, and Physical Disabilities. You will broker services, monitor outcomes, and support clients to live independently in their home and community.

Responsibilities include assessments, creating Person-Centered Plans, provider coordination, and ensuring confidentiality.

Qualifications

BS/BA degree in related coursework.
Experience with Microsoft Office and state-maintained databases.
Maintain proficiency in company-sponsored training & certifications.
CPR/First Aid certification and updated state Clearances.
Complete required training courses per program regulations.
Valid driver's license with personal vehicle insurance and current registration.
Notify supervisor of changes in auto insurance or license status.

Responsibilities

Complete assessments to identify participant needs and desires for service delivery.
Develop a Person-Centered Service Plan and update annually or with significant changes.
Assist participant in choosing providers to enable informed decisions.
Ensure Behavioral Support Plan and Crisis Plan align with ISP.
Report any incidents of abuse, neglect, or exploitation immediately.
Coordinate with other care coordinators to ensure seamless service delivery.
Research and develop alternative solutions to participant needs.
Maintain confidentiality of sensitive information.
