Job summary

Care Lync is seeking a full-time Case Manager to coordinate care forNevada-based participants enrolled in Medicaid waiver programs. The role emphasizes organization and attention to detail, with opportunities for bonuses and excellent benefits in a family-oriented environment.

You will work to discern needs, develop individualized plans, and maintain strong working relationships with participants and their families while ensuring seamless coordination among care providers.

Qualifications

• BS/BA degree in related course work.
• No prior experience required.
• Experience with Microsoft Office, state-maintained databases.
• Maintain updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage and current vehicle registration.

Responsibilities

• Discerning the disabilities and accompanying needs of participants and helping them achieve their goals.
• Develop and modify each participant's plan annually or after significant medical or social changes.
• Communicate with participants, families and providers to coordinate care across physical, behavioral and support services.
