Job summary

Integrity House in Morris Plains, NJ, is seeking a Case Manager for the Pathways to Recovery program to coordinate enrollment, assessments, IEPs, and supportive services for participants.

You will collaborate with employers, One-Stop Career Centers, training providers, and community partners to remove barriers to employment and promote long-term success, while applying trauma-informed, strengths-based approaches.

Qualifications

• Bachelor’s degree in a related field required; Master’s preferred.
• Minimum two years of case management or related human services experience.
• Experience with substance use disorders and justice-involved populations preferred.
• Knowledge of community resources and workforce systems.
• Strong organizational and leadership abilities; excellent communication skills.

Responsibilities

• Coordinate enrollment, assessments, IEPs, and supportive services for participants.
• Maintain caseload, conduct regular case management sessions, and address barriers.
• Coordinate transportation, childcare, housing referrals, and other supports to remove barriers to employment.
• Refer participants to behavioral health, healthcare, education, and community programs.
• Apply motivational interviewing, trauma-informed care, and strengths-based approaches.
