Job summary

RHA Health Services, LLC is seeking a Qualified Professional to coordinate, monitor, and support person-centered services for individuals with IDD in an ICF setting in North Carolina. You will lead the development of PCPs, manage admissions, and ensure bed occupancy through effective referrals and intake processes.

The role emphasizes collaboration with nursing, direct support staff, clinicians, and guardians to ensure regulatory compliance and positive outcomes.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field.
• At least 2 years of experience (in North Carolina) with IDD; 1 year required if Master’s degree is held.
• Experience in developing, writing, and implementing Person-Centered Plans.
• Working knowledge of ICF regulations, Medicaid/HCBS rules.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Lead admissions and census management, including pre-admission screenings and PCP development.
• Coordinate all aspects of admission, ensuring timely documentation and smooth transitions.
• Serve as primary IDT coordinator, scheduling and documenting interdisciplinary meetings.
• Monitor PCP fidelity and progress toward outcomes across clinical and personal goals.
• Maintain communications with families, guardians, and external stakeholders; advocate for individuals served.
