Hampton-Newport News Community Services Board seeks an experienced Support Coordination Supervisor to lead a team serving adults with IDD. The role blends staff supervision with program oversight to ensure responsive, person-centered services and quality outcomes.

You will guide seven coordinators, manage service plans and records, and ensure compliance with state and federal requirements while fostering collaboration with individuals, families, providers, and community partners.

Qualifications

• Bachelor's Degree in a human services field (Social Work, Psychology, or Counseling) with 3 years of experience in support coordination services.
• Experience supervising and developing a team of professionals (approximately seven Support Coordinators).

Responsibilities

• Lead, coach, and supervise a team of Support Coordinators, including caseload assignments, performance feedback, training, and professional development.
• Oversee person-centered planning, assessments, service plans, required contacts, and clinical documentation to ensure quality and timely service delivery.
• Ensure compliance with DBHDS, Medicaid Waiver, DOJ Settlement Agreement, IDOLS, SIS, and agency requirements.
• Provide guidance on complex cases, crisis intervention, advocacy, hospital coordination, discharge planning, and continuity of care.
• Support overall program operations through quality improvement, budget monitoring, program evaluation, and direct Support Coordination coverage when needed.
