Job summary

Qualifications
- Bachelor's degree in Behavioral Sciences or related Human Services field.
- Minimum of one-year experience working with the population.
- Basic computer skills; experience with assessment and service plan development preferred.
- Must have a valid driver's license, safe driving record, and reliable transportation to worksites, trainings, and meetings.
Responsibilities
- Provide support coordination and case management services to adults with developmental/intellectual disabilities.
- Assist individuals in accessing medical, psychiatric, educational, residential and other services.
- Collaborate with care teams to develop and monitor individualized service plans.
