Job summary

St. Mary's Home in Norfolk, VA seeks a Support Coordinator (QIDP) to lead the person-centered planning process, coordinate services, and ensure plans are implemented across Residential Services, DSPs, and clinical teams.

You will monitor ISP progress, collaborate with families and interdisciplinary staff, and ensure compliance with Medicaid regulations while upholding the organization’s values. The role requires strong communication, organizational, and leadership skills, with a focus on

Qualifications

• QIDP requires one of: a doctor of medicine or osteopathy licensed in Virginia, or a registered nurse licensed in Virginia, or a bachelor’s degree in a human services field.
• At least 1 year of experience working directly with persons having severe developmental and physical disabilities.
• Background in developing and implementing service plans.
• Current acceptable driving record and valid VA driver’s license.

Responsibilities

• Lead the ISP process and ensure plans are understood, implemented, and evaluated.
• Collaborate with the individual, family/guardian, Residential Services leadership, DSP Trainers, DSPs, and interdisciplinary team to coordinate supports.
• Document ISPs, progress notes, and required reports for department records.
• Provide staff training and coaching to ensure consistent implementation of plans across shifts.
