Job summary

RHA Health Services, LLC is hiring an IDD Qualified Professional to coordinate person-centered plans and manage admissions at an Intermediate Care Facility. The role focuses on fostering independence, wellbeing, and inclusion while ensuring regulatory compliance within the IDD and ICF framework.

You will lead the referral and intake process, supervise PCP implementation, and collaborate with nursing, staff, clinicians, and guardians to drive positive outcomes for individuals served.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field (Master’s preferred).
• 2+ years experience with IDD individuals (NC), 1 year (TN); Master’s degree may waive experience requirements.
• Experience developing, writing, and implementing Person-Centered Plans (PCP).
• Knowledge of ICF regulations, Medicaid/HCBS rules, and PCP principles.
• Strong communication, organization, and documentation skills.

Responsibilities

• Lead admissions and census processes, including pre-admission screenings and PCP development.
• Coordinate IDT meetings, translate client needs into PCP goals, and monitor progress.
• Conduct site visits to evaluate PCP fidelity and client well-being.
• Maintain communications with families, guardians, and stakeholders; advocate for individuals.
