Job summary

Care Lync is seeking a Support Coordinator to manage ID/DD Waiver coordination and the Plan of Services for individuals served. The role requires ensuring services meet health and welfare needs and coordinating with providers to secure appropriate supports.

The position emphasizes comprehensive documentation, compliance with DMH standards, and ongoing communication with legal representatives and supervisors to ensure timely enrollments and renewals.

Qualifications

• Bachelor's degree in a human services-related field required or
• Bachelor's degree in a non-related field plus at least one year of relevant work experience.
• Experience working with people with intellectual/developmental disabilities, mental illness or other disabilities.

Responsibilities

• Coordinate and monitor all services a person receives to ensure adequacy, suitability, and health.
• Conduct intake documentation for individuals referred to the agency.
• Track Medicaid and waiver eligibility and renewals for each person on caseload.
