24 Hour Home Care in El Segundo, CA is seeking a Community Supports Coordinator to guide caregivers through onboarding and first shifts, ensuring timely start of care and accurate timekeeping in FSL.

The role collaborates with Upstream Coordinators, members, and internal teams, managing documentation, verification, and admissions while maintaining HIPAA compliance. This is a hybrid position with monthly in-office presence in El Segundo.

Qualifications

• High school diploma or GED required.
• Minimum 1 year of customer service experience or experience in a service-oriented role.
• Experience supporting customers or applicants through multistep processes, including documentation collection and follow-up.
• Proficiency in Microsoft Office, including Word, Excel, and Outlook.
• Spanish-speaking ability preferred.

Responsabilités

• Manage assigned caregivers from the Upstream handoff through application completion, reviewing documentation, conducting consistent follow-up, and resolving outstanding requirements.
• Coordinate caregiver hiring requirements, including identification verification, employment eligibility, background checks, required approvals, and hiring readiness.
• Partner with members and caregivers to coordinate the first shift within 48 hours of hire, provide FSL training, troubleshoot access issues, and confirm accurate first-shift documentation.
• Support member admissions, caregiver rehires, and post-hire inquiries, collaborating with internal teams and Case Managers to address service concerns and ensure continuity of care.
• Maintain accurate caregiver and member records in Salesforce, track onboarding progress and outstanding requirements, and ensure compliance with company policies and HIPAA requirements.
