Job summary

Alleghenies United Cerebral Palsy (AUCP) seeks a Service Coordinator to partner with individuals and teams to create, update, and manage personalized service plans. This client-centered role emphasizes effective communication, coordination, and regulatory compliance across in-home visits and community programs.

Hybrid/Remote position with PA home visits; travel within PA is required. Prior experience with disabilities and strong planning skills are essential for success in this role.

Qualifications

• Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field.
• Knowledge of Medicaid Waiver programs, LTSS, and HCBS strongly preferred.

Responsibilities

• Develop, update, and manage individualized service plans with clients and teams.
• Coordinate and monitor services with paid providers and community resources.
• Maintain open communication with support teams and ensure compliance with AUCP standards.
• Travel within the assigned region to meet with participants and complete ISP requirements.
• Ensure proper documentation and billing per program regulations.
