Alleghenies United Cerebral Palsy (AUCP) seeks a Service Coordinator to develop, update, and manage individualized service plans for participants, coordinating paid providers and community resources to meet goals and needs. This client-centered role emphasizes communication, planning, and compliance with program rules.

Hybrid/remote work in PA with occasional home visits, requiring reliable internet and willingness to travel within the state; strong organizational skills and teamwork are

Qualifications

• Bachelor's degree in Social Work, Psychology, Education, Human Services, or related field.
• At least three years of professional case management experience for people with physical disabilities.
• Experience working with individuals with Autism is preferred.
• Knowledge of Medicaid Waiver programs, LTSS, and HCBS is strongly preferred.

Responsibilities

• Develop individualized service plans (ISPs) with clients and teams.
• Regularly update service plans to reflect life changes.
• Coordinate services with paid providers and community resources to ensure needs are met.
• Maintain open communication with support teams and ensure timely documentation and billing compliance.
• Travel within PA to meet participants and complete home visits.
