Zusammenfassung

Alleghenies United Cerebral Palsy (AUCP) seeks a Service Coordinator to develop, update, and manage personalized service plans in collaboration with clients and support teams. You will coordinate paid providers and community resources to ensure plans are followed and goals are met, with emphasis on client-centered care.

The role requires strong communication, organizational, and compliance skills and entails hybrid/remote work with PA home visits. Travel within PA may be required.

Qualifikationen

• Bachelor's degree in Social Work, Psychology, Education, Human Services, or related field.
• At least two to three years of professional case management experience for people with physical disabilities.
• Three years of experience working with individuals with Autism.
• Knowledge of Medicaid Waiver programs, LTSS, and HCBS preferred.

Aufgaben

• Collaborate with clients and teams to develop individualized service plans (ISP).
• Regularly update service plans to reflect life changes and current support needs.
• Coordinate services with paid providers and community resources to meet needs and ensure plan compliance.
• Travel within the region to meet participants as per ISP.
