Job summary

Alleghenies United Cerebral Palsy (AUCP) is hiring a Service Coordinator to collaborate with clients and teams to design and manage individualized service plans in a hybrid/remote PA role. You will coordinate paid providers and community resources to meet each person's needs and ensure proper implementation and billing.

The position emphasizes client-centered service delivery, strong communication, and documentation.

Qualifications

• Bachelor's degree required in a related field.
• Knowledge of Medicaid Waiver programs, LTSS, HCBS preferred.
• Strong communication, coordination, and documentation skills.

Responsibilities

• Develop individualized service plans (ISP) with clients and teams.
• Regularly update service plans based on changes.
• Coordinate services with paid providers and community resources.
• Maintain open communication with support teams and ensure compliance with AUCP standards.
• Travel within the PA region to meet with participants.
