Qualifications

• Knowledge of Medicaid Waiver programs and home-based services preferred.

Responsibilities

• Collaborate with clients to develop individualized service plans based on personal goals.
• Update service plans regularly to reflect life changes.
• Coordinate services with paid providers to meet individual needs.

Skills

Experience working with individuals with disabilities or special needs

Strong skills in assessment, planning, and participant-centered service delivery

Excellent organizational, communication, and documentation skills

Ability to work effectively as part of an interdisciplinary team

Familiarity with community support systems

Education

Bachelor's degree in Social Work, Psychology, Education, Human Services, or related field
