Qualifications

• Bachelor’s degree in Social Work, Psychology, Education, Human Services, or related field.
• Knowledge of Medicaid Waiver programs, LTSS, HCBS is preferred.

Responsibilities

• Develop individualized service plans (ISP) with clients and teams.
• Regularly update service plans to reflect changes.
• Coordinate and monitor services with paid providers and community resources.
• Maintain open communication with support teams for updates.
• Ensure compliance with program regulations and AUCP standards.
