Alleghenies United Cerebral Palsy (AUCP) is hiring a Service Coordinator. The role develops and manages ISP, coordinates services with providers, and ensures compliant documentation and billing. The position is hybrid/remote with visits in PA and travel within the region.

Ideal candidates hold a Bachelor’s degree in Social Work or related field, plus experience with disabilities, and strong organizational, communication, and planning skills. PA residency or willingness to travel is required.

Competenze

• Bachelor's degree required or related field.
• Knowledge of Medicaid Waiver programs, LTSS, HCBS preferred.
• Experience with individuals with disabilities or special needs is a plus.

Mansioni

• Develop, update, and manage individualized service plans (ISP).
• Coordinate and monitor services with paid providers and community resources.
• Maintain open communication with support teams and ensure compliance with AUCP standards.
• Travel within the region to meet participants and complete home visits in PA.
• Support billing, timelines, and documentation requirements.
