Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician to operate primarily in the field, traveling to client sites with occasional office attendance. The role supports the RN in case management, assessments, and care coordination for members with chronic illnesses and disabilities.

The position requires an LPN/LVN, LSW, LCSW, or LMSW license and at least 2 years of relevant experience. Hybrid/work-from-office expectations apply, with accommodations considered per policy.

Qualifications

• Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
• Current, unrestricted license as LPN/LVN, LSW, LCSW, LMSW or license other than RN in applicable state(s).
• May require state-specific certification based on state law and/or contract.

Responsibilities

• Assist the responsible RN in identifying members for high-risk complications.
• Obtain clinical data as directed by the responsible RN.
• Assist in identifying members that would benefit from an alternative level of care or waiver programs.
• Provide information collected to the RN for verification, interpretation, and care plan development.
• Coordinate care for members with chronic illnesses in alignment with the RN and health care team.
• Apply clinical guidelines to member needs; nursing judgment is not required.
