Job summary

Maximus seeks a Registered Nurse, Clinical Assessor to conduct person-centered assessments in homes, group homes, and care facilities, determining Long Term Care needs and placement options. The role requires commuting within and covering Franklin County and may involve on-site, video, or telephonic assessments according to program needs.

You will travel over 50% of the workweek, adhere to HIPAA and Person First standards, document outcomes, and participate in a broad benefits program including

Qualifications

• Minimum two (2) years of post-graduate nursing experience.
• Current RN license valid in the state of practice.
• Experience in long term care or home health care (preferred).
• Experience with OASIS or MDS (preferred).

Responsibilities

• Complete medical and/or behavioral health assessments using standardized tools and criteria in accordance with contract requirements; escalate complex or non-standard cases that fall outside defined protocols.
• Conduct on-site, video, and/or telephonic assessments based on program requirements to determine an individual's needs for services and supports, eligibility, and level of care based on established guidelines, seeking guidance from senior staff as needed.
• Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies; ensure accurate and consistent documentation of assessment outcomes.
• Travel to client, consumer or program locations to perform in-person assessments exceeding 50% of the workweek in support of program delivery.
