Job summary

Maximus in Massachusetts is seeking a Field Registered Nurse (RN) Clinical Assessor to join a hybrid remote program serving Franklin County. You will perform in-person health, functional, and social assessments in members' homes and community settings, developing person-centered plans and coordinating with providers.

The role requires MA RN licensure, 2+ years of post-graduate clinical experience, strong documentation skills, and familiarity with MassHealth LTSS.

Qualifications

• Active MA RN license required.
• Minimum 2 years post-graduate clinical experience.
• Experience with vulnerable populations and LTSS.

Responsibilities

• Complete medical and behavioral health assessments in home/community settings.
• Travel 50%+ per week to client sites; conduct on-site, video, or telephonic assessments.
• Develop and manage service plans with care teams and community resources.
• Ensure HIPAA compliance and confidentiality.
• Educate members, families, and caregivers on available services.
• Document assessments for billing and quality reporting.
• Advocate for members to maintain independence.
