Job summary

Maximus seeks a Field Registered Nurse (RN) Clinical Assessor in Massachusetts to conduct in-person assessments in members' homes and other settings, evaluating medical, functional, behavioral health, and social needs. You will develop person-centered service plans and collaborate with care teams to ensure compliant, high‑quality care.

The role requires travel within the state, 50%+ time, with Mon-Fri, 8:00 AM–5:00 PM scheduling and a hybrid remote work option.

Qualifications

• Active RN license in Massachusetts
• Minimum of two years of post-graduate clinical experience as an RN
• Experience with vulnerable populations (LTSS, behavioral health, I/DD)
• Experience conducting assessments in home or community settings

Responsibilities

• Perform medical/functional/behavioral assessments in member homes, hospitals, and community settings
• Develop and manage person-centered service plans
• Collaborate with healthcare providers, case managers, and community resources
• Educate members and caregivers on available services and options
• Document assessments for billing, compliance, and quality reporting
• Travel to client locations to perform in-person assessments (≥50% time)
