Job summary

Maximus is seeking a Registered Nurse (RN) Clinical Assessor for a hybrid remote position servicing Essex County, MA. You will perform comprehensive in-person assessments at members’ homes or community sites, develop person-centered care plans, and collaborate with healthcare providers to determine LTSS eligibility.

The role includes travel across the region and opportunities for professional growth. The ideal candidate holds an active MA RN license, has at least two years of post-graduate

Qualifications

• Active Registered Nurse license in Massachusetts.
• Minimum of two years of relevant post-graduate clinical experience.
• Experience with vulnerable populations and LTSS programs.

Responsibilities

• Conduct on-site, home and other assessments to determine service needs and eligibility.
• Develop and manage person-centered service plans with care teams.
• Educate members and caregivers on available LTSS resources and options.
• Document assessments accurately for billing, compliance, and reporting.
• Travel to client sites as required (over 50% of the week).
